Referral
Do you want to be eligible for reimbursement of your treatment? Then you need a referral. For a referral, you first make an appointment with your GP. You discuss your complaints and indicate that you would like treatment at Stroomlijn.
With a valid referral, the intake interview is always reimbursed. Even if it turns out after the interview that there is no diagnosis that qualifies for reimbursement.
The GP can best refer you to Stroomlijn via Zorgdomein (an online referral system).
You can also download a format below. You can print this out and take it to your GP. If your GP agrees with the referral, you can scan the letter or make a photo and simply upload it in our online client portal.
